Sliding Scale Membership Plan
Purpose: This program extends functional health care to individuals in recovery and financial hardship, ensuring access to root-cause solutions while honoring Portali’s mission of compassionate, science-backed healthcare. Eligibility: Must be referred through a recovery center, mental health program, or community partner. Proof of income status or financial hardship required. Limited number of spots available each year to maintain provider sustainability.
Tier 1: Sponsored Access (0–25% of Cost)
- For individuals with no income or active recovery support.
- Requires community service commitment in lieu of payment.
Tier 2: Reduced Investment (26–50% of Cost)
- For individuals earning less than 150% of the federal poverty line.
- Monthly membership investment adjusted proportionally
Tier 3: Supported Investment (51–75% of Cost)
- For individuals earning less than 200% of the federal poverty line.
- Membership priced at a reduced rate.
*Applicants must reapply annually. No supplements or lab testing included (self-pay only). Limited to a set number of reduced-rate spots per year.
